I agree to, consent to, and understand the following:

1. I am voluntarily participating in the BarrePop! exercise/fitness program conducted by Amber Robinson. I recognize that the program requires physical exertion that may be strenuous at times and may cause physical injury and I am fully aware of the risks and hazards involved.

2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the above mentioned program. I represent and warrant that I have no medical condition that would prevent my participation in the program. I will notify Amber Robinson immediately regarding any change in medical condition, including pregnancy.

3. I agree to assume full responsibility for any risks, injuries or damage known or unknown which I might incur as a result of participating in the program. Such injuries may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness, including death.

4. I acknowledge and agree that if I am pre-natal/postpartum I have a signed note from my Doctor agreeing to my participation in the program and will email Amber Robinson at the email address provided on this page. I understand that my participation in BarrePop! may increase the risk to myself and, if applicable, my unborn child. I assume all associated risks to myself, and if applicable, my unborn child.

5. I knowingly, voluntarily and expressly waive any claim I may have against BarrePop! or Amber Robinson for injury or damages that I may sustain as a result of participating in the program.

6. I, my heirs or representatives forever release waive, discharge and covenant not to sue BarrePop! or Amber Robinson for any injury or death caused by their negligence or other acts.

7. I verify that I am at least 18 years old.
